Providing Feedback

General Comments or Feedback: Please drop a note in the LSC Feedback/Suggestion Box. You can do this anonymously or add contact information if you like a response from team leadership. All suggestions are welcome. 

Safety Concerns: Please fill out LSC Incident Report. This form is designed to collect information about any event that impacts the safety or well-being of swimmers or their families. This includes events inside and outside facilities, with team members, and broader community alike. Issues with dealing with peer-to-peer bullying, coach-athlete bullying, parent issues, and violations of the LSC Code of Conduct may be reported here.

Other Reporting: Some concerns require external reporting. 

  • For issus dealing with sexual misconduct, sexual harrassment and/or sexually explicit or inappropriate communication through social media, please contact U.S. Center for Safesport at 833-5US-Safe(587-7233) or report online.
  • For issues dealing with physical, emotional abuse, criminal charges and the use, sale or distribution of illegal drugs, please contact USA Swimming Safe Sport at safesport@usaswimming.org or report online. 
  • For issues dealing with known or suspected child abuse, contact State authorities at 844-CO-4-KIDS(264-5437) or click here for more information.
